Online Consultation with Patients:
Our online, clinical consultation is created for those individuals who are relatively healthy and would like to speak with a physician regarding a general health assessment.
This would include the individual initially completing a general, private and discreet medical interview via telephone with our physician. The patient will be given a unique encounter number "xxxxxxxxx" at the initiation of our ONLINE MEDICAL SERVICES for PRIVACY OF YOUR HEALTH CARE INFORMATION. WE DO NOT DISCUSS THE PATIENT'S BIRTHDAY, SOCIAL SECURITY NUMBER, PROPER NAME OR OTHER IDENTIFYING INFORMATION via the internet or through our email system. Appropriate unique instructions as to the policy of MDC-ATLANTA, will be given to the patient at the time of initiating the private medical interview with our physician. A Physical Examination by our licensed physician/nurse practitioner may also be recommended as needed for further evaluation of your medical care based on the initial medical interview assessment.
Based on a review of your medical history assessment, you may be directed to visit us at our clinic on Cleveland avenue for a directed physical examination as well as recommendation of selected laboratory testing. Through our mobile medical care, our physician or a representative of our practice will come to collect a sample, if needed, from you while at your home or assisted/independent living facility to be submitted for selected medical laboratory testing. We won't order unnecessary lab tests like those other guys, because our goal is to be efficient and save you money. For example after our clinical assessment, if you don't have a clinical indication for a laboratory test, WE WON'T ORDER IT, and you can keep us to that promise. We will explain to our patients, step by step our medical strategy and decision prior to making them.
Directed, Medical Evaluation:
We evaluate patients for many different types of diseases including certain types of skin lesions (i.e. rash, skin cancer, etc), blood disorders (such as anemia, leukemia, lymphoma bleeding disorders), superficial lumps ( such as those of breast, thyroid, lymph node, etc), PAP smears, testicular/prostate exams, and much more, proven to save you time and money. We also diagnose and manage various types of chronic diseases including gastro-intestinal disorders, acute or chronic liver disease, high blood pressure, diabetes mellitus, lupus, arthritis, mixed connective tissue disorders, a variety of cancers, infections, and much more.
Our mobile medical care is second to none. Our associated registered nurse will visit you at your home or assisted/independent living facility for immediate evaluation of your sick care needs. Our experienced and skilled nurse interacts directly with the patient, assessing vital signs as well as other necessary physical medical evaluation on behalf of our mobile physician.

With an appropriately scheduled appointment, our mobile physician will also visit the patient at the patient's home or assisted/independent living facility to provide immediate medical evaluation and even diagnostic medical procedures and treatment in certain instances.
Hospital, Sick or Life-threatening Care:
We are an out-patient based medical practice. Thus, medical care of the patient's urgent, life threatening illness can be provided by the variety of emergency room physicians or hospitalists at the various community hospitals throughout Atlanta, or the surrounding towns/cities. Hospitalists are generally considered hospital-based physicians, typically internal medicine doctors, who oversee the patient's medical care while the patient is in the hospital only, and refer patients to the variety of specialists during the patient's in-hospital care. Then the hospitalist informs the patients primary care provider, office-based physician or nurse-practitioner, regarding the outcome of the patient's hospital care. The primary care provider is generally the physician/nurse-practitioner that helps you manage your day to day, non-life-threatening medical care. AND, that is the focus of our practice.

Here are the steps you would take to initiate our General Clinical Medical Practice Service as provided by MDC:
1) Call Dr. Gates to request a general health evaluation. After you have given Dr. Gates a general verbal consent to provide medical care to you, and you have acknowledged the privacy of the conversation between you (the patient) and Dr. Gates, Dr. Gates will perform a preliminary medical assessment of your needs for further medical evaluation.
2) Based on Dr. Gates' preliminary medical assessment or mobile care evaluation, Dr. Gates will advise you with regards to appropriate medical laboratory testing. Various patient-directed laboratory testing facilities are available regionally and nationally, for our online patients to access. You might also consider visiting us at our practice location in Atlanta on Cleveland Avenue, for direct phlebotomy service or for diagnostic procedures such as skin biopsy with frozen-section (an immediate diagnostic procedure), fine-needle aspiration of lumps, pap-smears, and blood smears. The results from patient-direct access testing facility can be sent to Medical Diagnostic Choices, Atlanta, Georgia, for interpretation by Dr. Gates. You will be given a full assessment and plan of action for further diagnostic management and/or therapeutic care. For mobile medical care, our registered nurse and/or physician will come to assess your medical needs in the privacy of your home, and collect phlebotomy samples for further medical laboratory testing.
3) After consenting to allow Medical Diagnostic Choices to provide primary care and laboratory medical service to you, further instructions will be given to you at the time of initiating this patient-physician agreement. If you desire, a final medical evaluation report by Dr. Gates will be confidentially available to you in detailed simple, plain ENGLISH, as well as recommendation for further clinical evaluation and follow-up.
We strive to provide references from the current medical literature with regards to evidence-based support as to how OUR final interpretive medical diagnostic evaluations, as well as treatment and management strategies are derived.
Fee for Service (Cash Only MDC-Atlanta):
1. General, telemedicine initial assessment: $20 U.S. dollars
2. Uncomplicated general medical evaluation: $75 U.S. dollars
3. Complicated general medical evaluation: $130 U.S. dollars
4. Diagnostic Procedures: $175-$800 U.S. dollars, depending on procedure to be explained to patient
Note: Fees for laboratory services are separate and will be explained to patient prior to ordering tests.
